Applicants must currently be under treatment and suffering hair loss due to
treatments.

Requirements:
This foundation is created to
deliver free cranial prostheses only to those who cannot afford
to purchase it. A total household income lower than 30k a year is needed and
proof is required.
A Doctor’s note stating the patient is under chemotherapy
or any other treatment which
causes their hair loss is necessary in order to honor the application.
Income guidelines determined on individual case-by-case basis. Average top income is $30k/annually but compelling circumstances, family-size and medical condition will be considered.
We
respectfully reserve the rights to terminate this program if a shortness of
funds occurred.
